I am an Out-of-Network provider, and I provide a Superbill to request reimbursement directly from your insurance company.  I recommend calling the member phone number on your insurance card and asking the following questions:

1) Do I have out of network coverage?

2) If so, how much is my deductible?

3) What is my coinsurance rate?

4) What is your allowable rate for a 90837 psychotherapy visit? 

5) Where do I submit claims? 

6) What is the address for claim submission?
